Dr Garthwaite sees adults with a wide range of respiratory problems, with particular expertise in interstitial lung disease and pulmonary fibrosis.
- Interstitial lung disease and pulmonary fibrosis (including idiopathic pulmonary fibrosis)
- Connective tissue disease–related lung disease, including rheumatoid arthritis, idiopathic inflammatory myositis, systemic sclerosis, Sjögren’s syndrome and SLE
- Asbestosis, silicosis and occupational lung disease
- Hypersensitivity pneumonitis and smoking-related interstitial disease
- Sarcoidosis
- Post-COVID lung disease
- Breathlessness
- Chronic cough
- General respiratory conditions including airways disease, asthma and COPD
Investigations and tests
Investigations are recommended only where they are likely to add useful information, and are explained clearly before they are arranged.
- Imaging: high-resolution CT and chest X-ray, with careful review of previous scans where available
- Full pulmonary function testing: spirometry (with reversibility if appropriate), lung volumes and gas transfer
- Blood tests including full autoimmune screening
- Bronchoscopy and endobronchial ultrasound (EBUS) via affiliated colleagues
- Lung biopsy where required after multidisciplinary review
- Cardiopulmonary exercise testing
- Respiratory muscle testing
- FeNO and bronchial provocation tests for assessment of asthma
- Oxygen assessments and ambulatory oxygen testing
Treatment options
Treatment plans are individualised and based on the latest evidence, national guidelines and the patient’s own priorities.
- Pharmacological therapy including immunomodulation such as steroids, mycophenolate, azathioprine and other agents
- Anti-fibrotic drug therapy including nintedanib or pirfenidone where appropriate
- Inhaled therapy for asthma, COPD and airways disease
- Symptomatic management of breathlessness and cough
- Referral to physiotherapists for breathing pattern disorders and pulmonary rehabilitation
- Onward referral to pulmonary hypertension and other specialist teams where needed
- Coordination of care with the patient’s GP and other hospital specialists

About Helen
Dr Helen Garthwaite is a Consultant Respiratory Physician with specialist expertise in interstitial lung disease (ILD). She trained at Imperial College School of Medicine and completed her specialist training at leading centres including the Royal Brompton, Royal Free and Hammersmith Hospitals.
She is a Consultant at University College London Hospitals NHS Foundation Trust and Clinical Lead for the ILD Unit, where she leads complex multidisciplinary care and contributes to national and international research and clinical trials.
In her private practice, she offers specialist, thoughtful consultations grounded in evidence, experience, and a deep understanding of the individual.
Clinical approach
Dr Garthwaite's approach is holistic, personalised, and collaborative. She recognises that respiratory and in particular, interstitial lung diseases impact her patient's daily life, emotional wellbeing, and long-term plans.
Each consultation is unhurried and tailored to the individual with an approach anchored on a detailed medical history and understanding of the person's lifestyle, values, and priorities, integrating the latest evidence with what matters most to the patient.
Where appropriate, care is coordinated with other specialists to ensure a seamless, joined-up approach.
